What is the medical outcome of this?

Alright so to me this sounds fishy and like it was a fatal cocktail for my grandma. While at hospice, they gave her morphine (at a high enough dose that she sedated), ambien (while being sedated on morphine), and Ativan (again while she was sedated on the morphine). I would appreciate it if someone who is a nurse or doctor would comment and leave their position as their source. I would also appreciate if anyone had any similar experiences with a loved one at a hospice home. Thanks.
